Key Responsibilities / Duties:
Resident Assessment & Therapy Planning:
Conduct comprehensive assessments of residents’ physical, occupational, or speech-language needs.
Develop individualized therapy plans and goals in collaboration with the interdisciplinary care team.
Reassess residents regularly and adjust treatment plans to meet evolving needs.
Therapy Implementation:
Deliver therapy services directly to residents, including exercises, functional activities, ADLs, mobility, swallowing, cognitive, and communication interventions as appropriate.
Utilize evidence-based practices and therapeutic modalities to optimize resident outcomes.
Educate residents and families on therapy goals, progress, and home exercises.
Documentation & Compliance:
Maintain accurate, timely, and complete documentation in the resident’s medical record.
Ensure compliance with federal, state, and local regulations, including Missouri licensure laws and facility policies.
Prepare for and participate in audits, surveys, and quality improvement initiatives.
Collaboration & Staff Education:
Collaborate with nursing and care staff to integrate therapy into daily care routines.
Provide guidance and training to therapy technicians or assistants as appropriate.
Participate in interdisciplinary team meetings to communicate progress, recommendations, and care goals.
Safety & Equipment Management:
Ensure safe use of therapy equipment and adherence to infection control practices.
Maintain therapy areas and equipment in good working condition.
Identify and report safety hazards or maintenance needs.
Qualifications (Detailed):
Required:
Active and unrestricted license in Missouri for the respective discipline: PT, OT, or ST.
Minimum 1–2 years of clinical experience in a skilled nursing, long-term care, or rehabilitation setting preferred.
Knowledge of therapy principles, assessment tools, interventions, and documentation standards.
Strong communication, collaboration, and interpersonal skills.
Ability to work independently and as part of an interdisciplinary team.
Preferred:
Bachelor’s or Master’s degree in the respective discipline (BS/MA in PT, OT, or ST).
Certification in specialized therapy areas (e.g., LSVT for speech, NDT for OT/PT, wound management experience).
Experience with electronic health records (EHR) and therapy documentation systems.
CPR/BLS certification.
